What shared care is
A shared-care agreement is a formal arrangement in which a specialist and your GP share responsibility for your ongoing care. Typically, the specialist leads on diagnosis and treatment planning, while the GP supports ongoing prescriptions and monitoring.
How it usually works
- The specialist completes assessment and, where appropriate, initiates treatment
- Once stable, a shared-care agreement may be proposed to your GP
- The GP, if they agree, supports repeat prescriptions and routine monitoring
- The specialist remains available for reviews and specialist input
GP participation is voluntary
Shared care depends on your GP practice agreeing to take part, which is always at their discretion. We will keep this page updated as our services develop and regulatory arrangements are completed.
